A chronic wound is any wound that has not become smaller or healed after 4 weeks of traditional care.  Our clinical team works with your primary doctor or healthcare team for the right individualized treatment plan to get the patient back to living a full, healthy life.  We determine the underlying cause that has prevented a wound from healing, and set the patient on the path for success and prevention of reoccurrence.

We understand the importance of continuity of care, and ensure that patient notes are shared back to the referring provider so all parties are collaborative towards each patient’s healing process. 

When partnering with First Class Wound Care, your referral to us is for care of only the wound, and we assume treatment and responsibility for only the wound, while your clinic/agency continues treatment for all other aspects of that patient’s care if you want. 

If you are referring from an Acute Care facility and the patient needs Home Health treatment outside of our services, we will write orders and bring in one of our Home Health partners to provide care.  We ensure each patient gets the full care they need, whether it comes from us or another provider.
